Isane wears her silver-purple hair short with a few long strands with beads. She also wears the standard shinigami uniform.
She appears to be adept at kido (demon arts) since she was able to track and inform all seated shinigami and the drifters (Ichigo and his company) about the location of Sosuke Aizen when she and Retsu Unohana learned of his treachery. She used these kido techniques while Unohana healed Captain Toshiro Hitsugaya and Lieutenant Momo Hinamori. It is also possible that she is the second best healer in Soul Society given her rank as lieutenant in the 4th Division.
She is the older sister of Kiyone Kotetsu, one of the two third seats in the 13th division. Unlike Kiyone, she is generally quiet and observant, though she shares her sister's sense of loyalty. She looks to Unohana as a mentor and mother figure.
It is noted by Unohana that Isane suffers from frequent recurring nightmares. This sometimes used as material for jokes, as Isane's nightmares are often odd or whimsical in their contents. For example, she once had a nightmare involving fish cakes.
